Pet Access Registration
To get the PET ACCESS CARD, please present the following requirements:
1. Original updated vaccination record of your pet (original copy only; photo and scanned or soft copy is not allowed)
2. One (1) photocopy of your pet’s updated vaccination record
• Front page
• Innermost portion clearly showing your pet’s latest rabies Vaccination
3. Two (2) 1x1 colored photos of your pet
4. Accomplish and sign the PET ACCESS Application form at the Pet Registration Booth located at the Ground Floor Al-Fresco.
5. You may also download the Application form here, then email the accomplished form to firstname.lastname@example.org. Drop by the Pet Registration Booth at the Ground Floor Al-Fresco where you can claim your card.
6. Submit the requirements and accomplished PET ACCESS application form to the Pet Registration Booth.
7. Please surrender your PET ACCESS to the Pet Registration Booth upon expiration and for processing of renewal. Lost Pet Pass shall be charged a replacement fee of P100. Renewal fee of PET ACCESS Card will be Php 50.
8. The Pet Access Registration Booth will be available starting March 21(Tuesday). This will be open every Tuesdays and Fridays-Sundays at 2:00pm-5:00pm.
